    血流导向装置(Flow Diverter, FD)已成为颅内动脉瘤(Intracranial Aneurysm, IA)的重要治疗方法,而FD的金属覆盖率(Metal Coverage, MC)是影响动脉瘤治疗预后的最重要参数之一。应用计算流体力学(Computational Fluid Dynamics, FD)技术模拟不同FD参数对血流动力学的影响,为术前制定合理治疗策略提供了可行方法。方法:运用多孔介质模型模拟FD置入动脉瘤的过程,针对特定的FD(Tubridge)计算其自身特有的多孔介质动量源参数(渗透率与惯性阻力)初始值。比较不同动量源参数值(初始值的80%,90%,100%,110%及120%)的情况下血流速度、壁面切应力(Wall Shear Stress, WSS)、体积流量、瘤顶压力等血流动力学参数的变化,并进行多孔介质模型针对病人特异性颅内动脉瘤血流动力学参数的敏感性分析。结果:IA各血流动力学参数对多孔介质模型渗透率的敏感性为:载瘤动脉WSS>瘤体WSS>瘤顶压力,而各参数均对惯性阻力的敏感性较低。结论:应用多孔介质模型能够通过选择不同的渗透率参数模拟不同FD的MC,对不同MC的FD进行建模需要调整特定的渗透率设置。

    Abstract:

    introduction:Flow Diverters(FD)provide a new method for the treatment of intracranial aneurysms. A more reasonable FD embolization strategy is developed by using numerical simulation method based on computational fluid dynamics (Computational Fluid Dynamics, FD) before operation. The rupture of IA and perioperative complications can be effectively reduced by a higher metal coverage (MC) of FD at the neck of the aneurysm. Methods: The porous media model is innovatively used to simulate the process of FD implantation into aneurysms, and the initial values of its own unique porous media momentum source parameters (permeability and inertial resistance) are calculated for a specific FD (Tubridge).The changes of hemodynamic parameters such as blood flow velocity, wall shear stress (WSS), volume flow and pressure of aneurysm-roof were compared under different situation (initial values of 80%, 90%, 100%, 110% and 120%), and the sensitivity analysis of porous media model for hemodynamic parameters of patient-specific intracranial aneurysms was further carried out. Results: The sensitivity of the hemodynamic parameters of IA to the permeability of porous media model was as follows: > > ,but the sensitivity to inertia resistance was lower. Conclusions: To choose different permeability parameters is to choose different MC of FD, so it is necessary to adjust specific permeability settings to model FD of different MC.
