    摘要:

    目的 依据临床手术统计实例,探究双开窗分支支架不同程度植入病变胸主动脉后血流场的影响。方法 建立胸主动脉、胸主动脉覆膜支架以及分支血管覆膜支架,采用有限元计算的方法,对分支支架不同程度(5mm、10mm、15mm)植入病变主动脉进行分析,借助体外测速实验平台,进行实验验证。结果 分支支架不同程度植入胸主动脉对维持血流场稳定灌注存在一定规律,其中编号为D10-5组的分支血管血流灌注率处于良好水平,MAX TAWSS为最低水平的44.94Pa,为仿真结果最优组。结论 保持左锁骨下动脉(LSA)分支支架植入胸主动脉较短的情况下,适当增加左颈总动脉(LCCA)分支支架植入主动脉的深度,以获得更为稳定的血流场,为临床双开窗技术实践提供理论参考。

    Abstract:

    Objective To investigate the effect of double fenestration branch stents on blood flow after different degrees of implantation in the thoracic aorta based on clinical statistical examples. Methods The thoracic aorta, thoracic aorta coated stent and branch vessel coated stent were established, and the branch stent was implanted into the diseased aorta in different degrees (5mm, 10mm, 15mm) by finite element method. The experiment was verified by in vitro tachymetry experimental platform. Results There was a certain rule for maintaining stable perfusion of blood flow field with different degree of branch stent implantation in thoracic aorta, Among them, the branch blood perfusion rate of group D10-5 was at a good level, and MAX TAWSS was at the lowest level of 44.94Pa, which was the group with the best simulation results.Conclusions Under the condition that the left subclavian artery (LSA) branch stent implantation in the thoracic aorta is short, the depth of left common carotid artery (LCCA) branch stent implantation in the aorta is appropriately increased to obtain a more stable blood flow field, which provides theoretical reference for the clinical practice of double fenestration technology.
